XenMobile distributes Lookout for Work to any user groups included in the Lookout for Work Delivery Group you defined in Creating a Lookout for Work Delivery Group. By default, this is your Lookout for Work user group in XenMobile.

Citrix Secure Hub prompts users to install the app, after which they accept permissions in order to activate Lookout:

If the user declines permissions or closes the app, their device is still activated and secured in Lookout and in your MDM. Lookout cannot alert the user of issues without having device permissions, but it continues to report issues to the Lookout MES Console.
